1. Introduction Welcome to A Chance to Believe, LLC (“Company,” “we,” “our,” or “us”). By engaging our services, visiting our website, or using any of our resources, you (“Client,” “you,” or “your”) agree to the following Terms and Conditions (“Terms”). These Terms govern your access to and use of our mental health services, website, and related materials. 

2. Services Provided We provide professional outpatient mental health services, including but not limited to: • Individual therapy • Family therapy • Couples therapy • Consultation services • Therapeutic Supervision All services are provided by licensed or supervised mental health professionals, in accordance with state laws and professional ethical standards. 

3. Eligibility Clients must be at least 18 years old to enter into services independently. Clients under 18 require parental/guardian consent. We reserve the right to decline services when clinically or ethically appropriate, and will provide referrals if necessary. 

4. Confidentiality and Privacy • We comply with all federal and state laws regarding confidentiality, including the Health Insurance Portability and Accountability Act (HIPAA). • Information disclosed in therapy is private and will not be released without your written consent, except as required by law (e.g., suspected abuse, danger to self/others, court orders). • You are responsible for safeguarding your own privacy when using electronic communications (e.g., email, telehealth). Please review our Notice of Privacy Practices for more details. 

5. Client Responsibilities By engaging in services, you agree to: • Provide accurate personal, medical, and insurance information. • Attend scheduled sessions or provide at least 24 hours’ notice for cancellations (see Section 8). • Participate in treatment collaboratively and communicate concerns directly with your provider. 

6. Fees and Payment • Fees for services will be disclosed prior to the start of treatment. • We accept self-pay and EAP referrals. • Payment is due at the time of service unless other arrangements are made. • Outstanding balances may result in suspension of services until payment is resolved. 

7. Cancellation and No-Show Policy • Appointments must be cancelled or rescheduled with at least 24 hours’ notice. • Failure to provide notice may result in a late cancellation fee or no-show charge, which you must pay prior to next appointment can be scheduled. 

8. Telehealth Services • Telehealth is provided through secure, HIPAA-compliant platforms. • By using telehealth, you acknowledge potential risks of technology (e.g., connectivity issues). • You agree to participate from a private location to ensure confidentiality. 

9. Emergencies We are not a crisis service. In case of an emergency, call 911 or go to your nearest emergency room. You may also contact the Suicide & Crisis Lifeline at 988. 

10. Termination of Services We reserve the right to terminate services if: • The therapeutic relationship is no longer beneficial, • You fail to comply with treatment agreements, • Ethical or legal obligations require termination. Referrals will be provided when appropriate. 

11. Limitation of Liability Our services are intended to support mental health and well-being, but we cannot guarantee specific outcomes. By engaging in services, you acknowledge and accept this limitation. 

12. Intellectual Property All materials provided (handouts, resources, website content) are the property of A Chance to Believe, LLC and may not be copied, distributed, or used for commercial purposes without permission. 

13. Changes to Terms We may update these Terms periodically. Updates will be posted on our website or provided directly to clients. Continued use of our services constitutes acceptance of the revised Terms. 

14. Governing Law These Terms shall be governed by the laws of the State of South Carolina, without regard to its conflict of law provisions.
